Advanced Practice
Nurse (APN) — A Registered Nurse
who has received a master’s degree and is qualified
to practice nursing in a specialty role. He or she
may be a Clinical Nurse Specialist or a Nurse Practitioner.
An APN uses advanced knowledge and skills to assist
in managing and coordinating patient care. They
may also provide further education to patients or

Therapist — A specially trained
and licensed therapist that evaluates and treats
the difficulties caused by a physical illness in
performing daily tasks, such as the ability to work,
care for self and family, and participate in leisure
activities. |

Physical Therapist
— A specially trained and licensed
therapist that evaluates and treats the physical
limitations caused by a physical illness and works
with patients to improve and optimize strength and
motor skill. |

Speech Therapist
— A specially trained and licensed therapist
that evaluates and treats any difficulties a patient
may have with speech, language, thinking, voice
and swallowing. |

Social Worker
— A specially trained and licensed professional
that helps identify and facilitate patient post-discharge
issues and needs. |
